Registration
Patients wishing to register with our practice can do so by downloading and printing out the necessary forms below:
- New Patient Registration Form - GMS1
- New Patient Questionnaire
- Summary Care Record Consent Form
- Medication form
Alternatively, you can register online.
Once completed, bring all the forms to reception and you will be booked in for a New Patient Health Check. Our surgery is open Monday to Friday 8 am to 6.30 pm. Please note you can also get the above forms from reception; should you have no access to a printer. Fill out the forms, hand them back and the receptionist will process them.
Who is eligible to register
Healing Health Centre operates an Open List Policy and will accept you if you are moving to the area, moving to a new address that is in the practice boundary area than your previous address, babies born to parents registered to the Practice and close family members of the Practice patients living in the same address. We would also like to point out that the practice does not discriminate on the grounds of race, gender, social class, religion, sexual orientation, appearance, disability or medical condition

Expected behaviour policy and refusal to accept a patient
Safety of our staff and other patients is paramount. Therefore, we have the right to refuse any patient who is violent, abusive, drunk or visibly under the infulence of drugs. When these rules are not followed we have the right to withdraw from treatment of the patient and remove them from the premises.
